To Report a Workers' Compensation Claim

Submit a Claim

Note: Please submit ALL non-catastrophic Workers' Compensation claims online at NetClaim.net.

Before you start to report a claim, CIRMA recommends that you gather the information needed by first downloading, then completing our electronic Injury Reporting Information form, which may then be retained for your records. Please first download the form before filling it out. 

System & Browser Requirements:

  • Windows XP, Windows 7.
  • Internet Explorer v11 (with continued support of v10 in compatability mode, as of 06-06-2017.) System Upgrade:  Because we modified the protocols that allow you to create a secure connection with your NetClaim portal, some older browsers will no longer be able to connect. Attempts to access the Netclaim portal using a non-supported browser will result in loading or connection issues and an error message indicating that the browser cannot display the page or there is a problem accessing the site.   • Pop-ups and Cookies must be allowed to access the NetClaim Portal. Quick Tips to Allow Pop-Ups.

Please Note: All claims must be submitted by an authorized claims contact. To request a LOGIN ID and PASSWORD, please contact Sean Gaffney at sgaffney@ccm-ct.org, or at 203-946-3724. Please download, and then submit the Online Reporting Sign-Up form.

The Injury Care & Management Process

After an injury, your employee visits your local Initial Care Physician for treatment.

Then,

  • CIRMA nurse case managers validate the treatment plan.
  • Any required medical scans are provided by One-Call Medical centers.
  • Prescriptions are provided through the First Script Pharmacy benefit program with no out-of-pocket expense to the employee.
  • Crutches, braces, or other needed durable medical equipment are provided by the CIRMA team.
  • The injured employee is referred to a specialist within the network, if needed.

If his or her injury is catastrophic,

Then,

  • Catastrophic care specialists review the diagnosis, treatment plan, and care.
  • A CIRMA Field Case Manager advocates for the injured employee, coordinates care, and reviews the treatment plan.


If additional care is needed,

Then,

  • A Transitional Return-to-Work program is implemented until Maximum Medical Improvement.
  • Your employee returns to productive activity on the job, if possible.
